Copy of identity card (for Vietnam citizens in the country ) , passport or papers of replacement value , visa (for foreign citizens Vietnam and abroad ) of the recipient and who are recognized as parents ;
Copy of birth certificate of the father, mother ;
+ Papers , documents or evidence ( if any) between the receiver and demonstrate the recognize the relationship of father, mother , son ;
+ A certified copy of the household registration book or certificate demographic group (for Vietnam citizens residing in the country ) , permanent resident card (for foreigners residing in Vietnam ) of recipients the child .
